IMMIGRATION PHYSICAL EXAM
The US Citizenship and Immigration Services (USCIS) requires all aliens who are applying for green cards and citizenship to complete the I-693, the Report of Medical Examination and Vaccination Record. The purpose of the medical examination is to evaluate the applicant’s health status and to determine if any conditions exist that might prevent a change in status. This examination can only be conducted by a Designated Civil Surgeon like Dr. Todd Nguyen. He will complete and seal the I-693 form so that you can send the I-693 form back to the USCIS.
This exam requires applicants to be free of certain infectious diseases, including tuberculosis and syphilis. If Dr. Todd Nguyen finds that you have an infectious condition that would prevent you from passing the physical examination, she can treat you or recommend a treatment plan necessary to allow you to proceed with the next step in the immigration process.
What Do I Need To Bring To An Immigration Physical?
Please bring the following to your appointment:

Valid, government issued picture ID such as a valid passport, driver’s license, or other state-issued picture ID.

Prior medical records including any document history written in English or translated into English of vaccinations and/or documentation of treatment for tuberculosis.

I‐693 form: Fully complete the first page in black ink, with your name on all pages before you arrive.

Method of payment: We accept all forms of payment. Payment of $495 for the office visit and the mandatory lab required testing listed below. In addition, the cost of any vaccines given on the first appointment is due at the end of the first appointment.

If unable to speak, read and write English, you must provide an interpreter (over the age of 14) for the exam and the follow up appointment.

Please Note:

Insurance companies will not cover immigration physicals and/or vaccinations.

Please provide all vaccination record if they are available.

If you had a positive PPD (Tuberculosis) in the past, you will need a chest X-Ray.

If you are pregnant, please let the physician know so we can administer the allowed vaccinations only.
How Much Does the Immigration Physical Exam Cost?
The cost to complete the Immigration Physical Examination by the Physician is $195 and includes:

Physical Examination and completing the Civil Surgeon part of Form I-693.

Interpreting the test results by the Physician.

Reviewing and Signing your completed Form I-693 by the Physician.

Providing you certified and sealed copy of I-693 to submit to USCIS.

Providing you copies of your lab results (if requested).
Mandatory Required Laboratory Tests (for applicants with complete shot records): The cost for all the Immigration required mandatory laboratory tests is $300 and includes -

Syphilis Test (Rapid Plasma Reagin - RPR)(blood test)

Tuberculosis Test (QuantiFERON)

Gonorrhea Test (urine test)

Drug and Alcohol Screening Questionnaires and if you have a history of drug abuse, yo might be asked to take a drug test and or certify that you have completed a drug treatment program. If you're currently abusing prescription drugs, illegal drugs, or alcohol, you will not be allowed to get a green card. Drug Test (9 Drug Classes Urine Test)
The following additional services may be needed (for applicants with incomplete shot records or with other stipulated Immigration health-related conditions):
The cost is separate and varies depending on each applicant. Such services may include -

Any required missing Immunizations & Vaccinations (usually for the applicant with incomplete shot records).

Hepatitis B Testing (for the applicant with incomplete shot records).

Chest X-ray (if necessary based on test result). $75

If necessary, the applicant may be required to follow up with a specialist or a County Health Department to treat and clear the applicant of certain health-related condition mandated by Immigration before proceeding to the next step of the application process.
